Use this form to submit eligible items outlined in the KMS Reimbursement Policy.

For team staff trainings, include the receipts for courses or certifications that were/would have been submitted at the beginning of the season for Team Staff Qualifications. All receipts need to be matched to proof of completion/passing of required courses
All payments from the club are sent at the end of the current season if requests are eligible and complete.

Provide Details (information that may be needed for the club to approve your request, such as, for what courses, expenses, etc. the receipt are for). Being specific helps the club volunteers know if the request is eligible for reimbursement

Note regarding Documentation:

(1) Proof of PAYMENT (a receipt for courses showing amount paid)

(2) COMPLETION PROOF (certificate or email) of a course. For the Coaching Soccer in Canada module only there is no completion certificate unless you do the full age specific course it is part of, so receipt of payment is all that is needed.
